There are two components of a Pap Smear: 1) obtaining the specimen and 2) analyzing the specimen

1) Codes used for obtaining the Pap Smear specimen. CPT does state that a Pap smear is part of the preventive service.

99381 – Initial comprehensive preventive medicine evaluation and management of an individual including an age and gender appropriate history, examination, counseling/anticipatory guidance/risk factor reduction interventions, and the ordering of laboratory/diagnostic procedures, new patient; infant (age younger than 1 year)

For Medicare patients, annual wellness visits bill the following code with the Q0091 if pap smear is obtained:

G0438 – Annual wellness visit; includes a personalized prevention plan of service (pps), initial visit


G0439 – Annual wellness visit, includes a personalized prevention plan of service (pps), subsequent visit

Q0091 – Screening papanicolaou smear; obtaining, preparing and conveyance of cervical or vaginal smear to laboratory

G0101 – Cervical or vaginal cancer screening; pelvic and clinical breast examination
